Medication Management Program in Partnership with Partners In Care FoundationHomeMeds medication safety program is an evidence-based program that is proven to improve medication use and reduce duplication.
- Do you see multiple doctors?
- Do you fill prescriptions at multiple pharmacies?
- Have you recently experienced a fall, dizziness, or unusual confusion?
- Have you recently been to the ER or hospital?
- Do you take multiple medications?
- Do you take blood thinners or diabetes medications?
- Do you take over-the-counter drugs or supplements?
- Do you take medication to help you sleep?
This program is a free review of all your medications by a pharmacist. If you answer "Yes" to some or all of the following questions, you may benefit from a review. During a review, a staff member visits with you at home to prepare a list of all your medications, including over-the-counter drugs and supplements. A trained pharmacist then reviews this list to identify any problems and interactions. The pharmacist will work with you and your doctors to make sure you are only taking the medications that are right for you.
